Reading Hospital Fall Internship – Grant Community Connection Specialist

The Community Connection Specialist will be responsible for screening patients for social needs and providing them with information and referrals to community services. Qualifications

High School Diploma or GED (required)
Interest or Major in Health Care related field (preferred)
Experience and Skills

Interviewing techniques, self-advocacy, self-direction
Parent/Family engagement
Ability to assess patient need(s)
Knowledge of community specific resources
