Berks Encore Care+ is a new program, designed to provide invaluable assistance to individuals and their families who need help during challenging times. By working with an Aging Life Care Manager, you can transform an overwhelming experience into one that rallies resources and helps you feel your needs and preferences are being dealt with in the best possible way.
How does it work? After a complimentary phone consultation, the Aging Life Care Manager will determine the best way to help by doing a comprehensive assessment on how the client is functioning at home, document any medical conditions and treatments already in place, evaluate financial resources and recommend ways to improve daily functioning. The client and family, together with the Life Care Manager will agree on a plan of care to maintain or improve the client’s quality of life and provide peace of mind for the family. There is a flat fee for the assessment and care plan. Ongoing assistance to implement the plan can be arranged for an hourly fee.

Berks Encore Care+ can be a resource for people whose families are close by or far away, or who have changing circumstances or complex problems that are difficult to manage on their own.
After the comprehensive assessment and personalized care plan is created, the client and their family can execute the care plan recommendations on their own or engage ongoing assistance for an hourly fee.
