What is the Be Well Berks Grant Program? The purpose of the “Be Well Berks” Grant Program is to address the priorities identified in the 2019 Community Health Needs Assessment (CHNA).
A Be Well Berks award is a small, non-recurring grant designed to provide funds to start a small project or assist with the expansion of an existing project.
Applicants are encouraged to read both the 2019 CHNA and Implementation Strategy. Those documents can be found here. Applications will only be considered if the project closely aligns with the needs identified in the CHNA. Priority will be given to organizations that propose new, innovative, and collaborative strategies to address social determinants of health, specifically food insecurity.
Reading Hospital is not using the traditional funder-grantee model; rather, is seeking to be a partner. Successful applications will detail Reading Hospital’s role as a collaborative partner.
